Club Form - Self Declaration and Disclosure
As part of the safer recruitment of volunteers, clubs should carry out checks on volunteers to ensure they are suitable to carry out roles or tasks at your club. It is recommended that clubs should request all volunteers to complete a Self Declaration and Disclosure Form prior to starting to volunteer.
Licensed roles (coach/officials), Welfare Officers and Team Manager will also need to complete a DBS check.
